You play as a lamb who is about to be sacrificed by four powerful Bishops who rule over the Lands of the Old Faith. Just before your death, you are saved by a mysterious chained deity called The One Who Waits. This entity gives you a Red Crown that grants you special powers and a second chance at survival. In return, you must build a cult in his name and gather followers who will worship him. Your mission is to venture into four dangerous regions controlled by the Bishops, defeat them one by one, and free The One Who Waits from his chains. Along your adventure, you meet Ratau, a previous vessel who wore the Red Crown before you, and he teaches you how to manage your cult and survive in this dangerous land.

The game splits into two main parts. First is the dungeon crawling section, where you go on crusades into randomly generated dungeons across four regions. You fight enemies using various weapons like swords, axes, hammers, and daggers. During these runs, you also collect tarot cards that give you temporary power-ups, find curses that work as special attacks, and gather resources like wood, stone, and food. When you die, you lose some resources. The second part happens at your cult base, where you build structures, manage your followers, and perform rituals. You construct buildings like sleeping quarters, farms, cooking stations, and ritual circles. Your followers need food, shelter, and faith to stay loyal. You must give sermons to increase their devotion, perform rituals for special benefits, and sometimes sacrifice followers for greater power. Followers can get sick, grow old, and die, so you must take care of them properly.

The Relics of the Old Faith update added new buildings, fleeces that modify gameplay rules, and a post-game mode where you can resurrect and recruit the four Bishops as followers. The Unholy Alliance expansion brought local cooperative multiplayer, where a second player controls a Goat character.
