The events of Killing Floor 2 take place just one month after the original game. A company called Horzine Biotech conducted experiments that went very wrong, and the resulting creature clones, called Zeds, have spread rapidly across continental Europe. Governments have collapsed, communication has broken down, and military forces have been wiped out. A group of civilians and mercenaries has come together to fight back against the Zed hordes by taking on missions in infected areas across Europe.

Killing Floor 2 is a wave-based survival shooter where players face increasing numbers of Zeds across multiple rounds, with a powerful boss-type enemy appearing at the final wave. The game supports up to six players in online co-op and also has a solo mode. Players choose a perk — which is a character class — at the start of a match, and this determines their strengths and the weapons they can use most effectively. Perks include classes like Commando, Medic, Berserker, Firebug, and more. As players earn experience, their perks level up and they can pick from different skills.
The game also includes a versus mode where one team plays as humans and the other controls Zeds. Melee combat uses directional attack and block inputs, making close-range fights feel more active. The game features a slow-motion mechanic called Zed Time, which activates during intense moments.
